The clinical application is mainly for the treatment of coronary heart disease; Prevention of angina; Treatment of persistent angina pectoris after myocardial infarction; In combination with digitalis and/or diuretics to treat chronic congestive heart failure; Treatment of pulmonary hypertension.

The main pharmacological effect of isosorbide nitrate (ISDN) is to relax vascular smooth muscle. ISDN is metabolized in the body to produce isosorbide mononitrate (ISMN), which releases nitric oxide (NO), which is the same as endothelial relaxation factor, activates guanylate cyclase, increases the cyclic guanylate (cGMP) in smooth muscle cells, thus relaxing vascular smooth muscle, dilating peripheral arteries and veins, and having a stronger effect on venous dilation. Venous dilation results in peripheral blood retention, reduced cardiac return, left ventricular end-diastolic pressure and pulmonary capillary wedge pressure (preload). Arterial dilation reduces peripheral vascular resistance, systolic arterial pressure and mean arterial pressure (afterload). Coronary artery dilation increases coronary perfusion volume. The overall effect is to reduce the oxygen consumption of the heart muscle, increase the oxygen supply, and relieve angina pectoris.
